On the subject of safety and management

Jim,

My concern and observations are more dangerous and blatant at the mill manager and superintendent level where the operations and maintenance staff hear and sometimes more important, perceive that safety is less important than production. It only takes a little of this perception to cause someone to not take the time for a lockout/tagout that will require much more time than it will take to do the repair. Failure to LOTO is a leading cause of fatalities. Automated handling systems at the dry end are frequent locations that this has happened. Equipment with multiple control panels are another. Thanks for doing this series.
